CANCELLATION POLICY
Please remember to cancel or reschedule at least 48 hours in advance via the Client Portal. Late cancellations & No Shows will be charged $100.
If your appointment is at 2pm on Thursday you need to cancel before 2pm on Tuesday. Log in to the Client Portal to cancel—do not text Jessica unless you need to Late Cancel.
Enable email & text alerts for appointment reminders.
If you are late for a session, Jessica is unable to go over your allotted time as she needs to prepare for her next session.
A $25 service charge will be charged for any checks returned for any reason for special handling.
BILLING
Sessions are billed at the time of service. A credit cared is required to hold your appointment. You can pay via cash, check, or credit card. Health Savings Accounts or Flexible Spending Accounts (HSA/FSA) are accepted and cover Occupational Therapy services so be sure to add that debit card to the client portal or bring it to your session.
Transform Myofascial Release is an Out-of-Network Occupational Therapy provider. We do not bill your insurance provider. Some insurances provide out-of-network coverage. We will provide you with the necessary receipts and documentation if you would like to pursue reimbursement directly through your insurance provider.
Transform Myofascial Release, Inc. is not a Medicare provider. If you are 65 or older you have Medicare & may be able to access therapy elsewhere. It is up to you to seek out that option. By signing below you agree to not submit receipts from Transform Myofascial Release, Inc. to Medicare for reimbursement.
PREPARING FOR YOUR SESSION
The standard meeting time for Myofascial Release sessions is 60 minutes. Please refrain from wearing heavy lotion on your skin. Please bring something to change into so that we can see your body structure and have as much access to your skin as possible. Ideally, this would be shorts for men, and shorts and a bra or two piece bathing suit for women. However, if this isn’t comfortable for you, bring something comparable ie. tank top and shorts.
Your comfort is important to us. Please let us know if you are hot or cold, etc.
